Service Applications Processing

Louisville Water’s Service Applications department processes domestic, irrigation, and fire service requests for residential, commercial, industrial, and multi-family entities within new or existing developments. Please review all sections below prior to submitting an application for new service.

  • Click on the below New Service Application button and complete all the fields listed in the form.

Review the Documents Required section to ensure all necessary attachments are included. The application will not submit unless all fields indicated with an * are completed. Debit/Credit card or EFT bank draft are the acceptable payment methods for electronic applications. There is a 2.5% processing fee assessed to all card transactions. Please note, payment for electronic submittals will not take place until after the application has been processed. Payment is due in full before the application will move to our installations department

Documentation Requirements

Plumbing Permits -

  • Plumbing Permits – A copy of the approved permit is required for each domestic and irrigation application
  • Plot/Site Plans – All applications, with the exception of irrigation retrofits only, are required to include a site/plot plan with the proposed meter location marked.
